The complex nature of kidney disease information, low awareness and limited access to chronic kidney disease (CKD) information is a major barrier. There is also limited time availability within routine clinic visits, which is the most commonly cited barrier to CKD education. Patients most frequently feel undereducated and under supported in this arena. We help patients to implement the necessary activities and participate in care decisions and planning, providing them with CKD education suited to their unique needs and self-management support (SMS).
